As we age, everyday tasks become more challenging. We may need assistance with personal care, medication management, meal preparation or transport getting to and from medical appointments.

Many people are unable to drive due to medical factors such as poor vision, or may no longer have the confidence to navigate traffic. Family members or friends may not always be available to assist.

If you have any doubts about your loved one attending important medical visits on their own, or would love them to be part of a special family occasion, our reliable nurse assisted transport service is available seven days a week to help.

We transport people from their homes or aged care facility.

The fully equipped, air-conditioned and specially modified vehicles and equipment offer comfortable door to door travel across Sydney
and interstate. Mobility aids and equipment are available to ensure the most comfortable and tailored care is delivered during transportation.

Thanks to IMB Bank through the IMB Bank Community Foundation, close to 1,000 seniors living in and around the Sutherland Shire will benefit from the installation of new and more individualised equipment for three Clever Care NOW assisted transport vehicles.

Our office is based in Sutherland Shire but can travel across Sydney, New South Wales and interstate.

Hospital Transport

Clever Care Now provides a reliable, safe and flexible service to transport people to and from hospital pre admissions and following discharge.

If needed, our home nursing team is available to provide continuity of care at home during recovery.

Upon discharge we can offer six weeks post-acute nursing care without charge, for those eligible. We also offer nursing care through approved Aged Care Package providers and to private patients.

Medical appointments

Our nurses can liaise with doctors, specialists and health provider clinics to ensure ample time is scheduled to arrive at appointments on time.
The door to door service allows travel to and from home, aged care facilities or nursing homes to the chosen health professionals.

Social outings

Being socially active and participating in the community is vital for overall health and wellbeing. Our assisted transport service is available for shopping trips, lunch, dinner and other social outings.

Events

We provide the best possible level of care to help elderly or wheelchair bound loved ones attend special occasions such as weddings, birthdays, funerals and special events.

There are many things that make a wedding special and none more so than having a grandparent, elderly or wheelchair bound loved one with you on the day.

We can transport grandparents or elderly loved ones to weddings in our specially modified vehicles accompanied by professional and qualified nurses to offer extra assistance if required.

We plan the assisted travel with the bride, groom or family to ensure the utmost care is taken for the loved one’s travel, access to the venue and ongoing care throughout the celebration if required. We can travel across Sydney, New South Wales and interstate.

Funerals

Farewelling family or friends at funerals is an emotional time. We help families ensure their loved ones can be part of that time, remembering and honouring those most dear to them. Our caring staff provide transport and offer support during the time of bereavement.

Final wish

We offer transport for those nearing the end of their life to visit family or a special place. We can help make their final wish, one to be cherished by all. Supported by our professional nurses, we take care to ensure comfort during these special moments.
